We will be running a prior authorization and getting your in or out of network
coverage.
Depending on your coverage The $200 consultation fee will be billed to insurance
and $200 of each infusion which is ($500) Cash.
NOW: you pay $300 cash per infusion, and the consult ($200) , and each infusion
($200) will be billed
